St. Jude is seeking a compassionate Nurse Care Coordinator to support adolescents and young adults with sickle cell disease as they transition from pediatric to adult care. In this vital role you will provide education care coordination and emotional support to patients and familiesbeginning during late adolescence at our hospital and extending through their first visits at partnering adult sickle cell centers - to promote a safe smooth transition reduce complications and empower young adults to manage their health with confidence.
The Nursing Care Coordinator fosters the effective navigation of the healthcare continuum for our patients and their families. This role is grounded in relationship-based care and the SJCRH mission and values which maintains a primary and consistent focus on evidence-based quality of care safe transitions timely access to and availability of services and most importantly client self-determination and culturally-relevant care.
The SJCRH Nursing Care Coordinator enhances the experience and optimizes care delivery of the patient through transitional periods of care by effective navigation of the healthcare continuum. This role is academically and experientially prepared to function independently in accordance to the scope of practice in nursing demonstrating current knowledge skills and competence to effectively provide holistic and patient/family-centered care and leads the interdisciplinary plan of care through assessment planning collaboration evaluation and advocacy of comprehensive care plans that meet the needs of the patient and families served.
Minimum Education and/or Training:
- Bachelors Degree required.
Minimum Experience:
- Minimum requirement: 4 years of relevant experience.
- Solid understanding of the patient care system at St. Jude.
- Some experience using specialized equipment and patient education materials for the care of the pediatric oncology and hematology patients preferred.
- Some experience recording and maintaining data on educational activities preferred.
- Working knowledge of infection control policies and universal precautions.
- Proven performance in earlier role.
Licensure Registration and/or Certification Required by Law:
- (LC: RN TN) Must possess a current Tennessee State Board of Nursing license if primary residence is Tennessee or a Nurse Licensure non-Compact state.
- (LC: RN ARK or RN Miss) Must possess a current State Board of Nursing license in the state of primary residence if the state is a Nurse Licensure Compact state.
Compensation
In recognition of certain U.S. state and municipal pay transparency laws St. Jude is including a reasonable estimate of the compensation range for this role. This is an estimate offered in good faith and a specific salary offer takes into account factors that are considered in making compensation decisions including but not limited to skill sets experience and training licensure and certifications and other business and organizational needs. It is not typical for an individual to be hired at or near the top of the salary range and compensation decisions are dependent on the facts and circumstances of each case.
