The role of a Case Manager is to work with adults (ages 21 and up) to identify needs and goals that will strengthen the adults future. Areas of focus include but are not limited to mental health physical health economic needs education and positive social development. Clients are met with weekly on average and work towards these goals together in the community where the needs are as well as in the home itself. Staff transport clients when needed to make sure important connections to community resources are successful. The combined efforts of the case manager and the informed client led approach deliver some of the best outcomes in the field of human service work. Its challenging work for sure but the outcomes and rewards of changing generational cycles and improving the quality of lives in our communities outshines everything else.

Job Type: Full-time
Salary: $39802.88 AND UP per year (depending on experience and educational attainment)

Experience:
- Adults: 1 year (Required)

Shift availability:
- Day Shift (Required) - but may require some evenings (case-to-case basis)
Work Location:
- In person
- Minimum of 1 year adult case management/social work experience (or related experience) - volunteer/internship experience may be counted
- Minimum of 4-year degree (related human service degree preferred)

Potomac Community Services (PCS) due to its human services responsibilities and sensitive work is obligated to maintain a Drug-Free Workplace.
