You will be updated with latest job alerts via emailIOCP is a nonprofit that provides services in food case management emergency financial assistance employment services and childcare resources to those residing in eight Western Hennepin County cities. We are looking for a Neighborhood Program Case Manager to play a key role working independently and onsite with residents in 2-3 housing communities providing individual services and community connection to residents.
The Neighborhood Program Case Manager conducts new client intakes provides information and referrals does 1:1 case management and creates meaningful community connections. Work location: primarily based at assigned housing sites and the IOCP office with some flexibility to work remotely

What You Will Do
Conduct client intakes with a focus on exceptional customer service and quality data collection through trauma-informed practices. Describe our services collect information and assess presenting needs
Provide information and referrals responding to the needs of clients with a trauma-informed approach and facilitate their connection to internal and external programs and resources. Assist residents in crisis by offering immediate support and connection to appropriate resources
One-on-one case management including resolving crises assessing needs connecting to resources developing and implementing personalized plans to increase stability and achieve individual goals. Work with residents to assess strengths and barriers. Provide ongoing support through advocacy connections to external and internal resources counseling and problem-solving assistance in support of housing retention and stability goals
Maintain regular communication with property management to collaboratively address resident and community needs including resolving conflicts discussing account balances and planning upcoming events and initiatives.
Promote a strong sense of community through needs assessments and the organization of community-building events. Collaborate with residents volunteers property managers and partners to coordinate community dinners resource fairs youth activities and other engagement events.
Schedule includes attendance at evening programming and events to ensure success approximately 23 times per month with flexibility as needed

What You Will Bring
BSW preferred or Bachelors degree in a related field
3-5 years direct service experience working with adults and families
Experience working in housing communities
Excellent interpersonal verbal and written communication skills
Demonstrated experience working with populations of diverse age income race national origin and disabilities
Commitment to equity and inclusion and ability to excel in a diverse environment
Knowledge of and ability to navigate community resources and systems
Ability to work independently as well as effectively in teams
Self-motivated and able to work well with limited supervision
Excellent organization and time management skills
High level of computer proficiency
A valid drivers license with a clean driving record and access to reliable transportation
Must be willing to travel between work sites
Ability and willingness to transport program participants in the Neighborhood Program van
