GENERAL DESCRIPTION:
This position serves as a Social Worker for the Skilled Nursing Facility (SNF) at the Wyoming Life Resource Center (WLRC) in Lander. Team members enjoy an excellent compensation and benefits package!

ESSENTIAL FUNCTIONS: The listed functions are illustrative only and are not intended to describe every function which may be performed at the job level.- Work with the Interdisciplinary Team and Administration to promote and protect resident rights and each residents psychosocial well-being.
- Work with resident families and/or guardians to provide support information and education.
- Work with the Admission/Discharge Coordinator to facilitate SNF admissions discharges and transfers to ensure all documents and plans are in place in accordance with federal and state regulations.
- Clinically assess the residents mental health and complete assessments that identify social emotional and psychological needs.
- Participate in the resident care planning process in developing an interdisciplinary plan of care for each resident.
- Monitor and ensure that resident psychosocial needs are being met including treatment by staff resident-to-resident interactions and opportunities for community activities.
- Work with all stakeholders such as the Wyoming Department of Health Behavioral Health Division Wyoming State Hospital Aging Services Division and community nursing home providers to determine eligibility and proper placement for persons served.
- Participate in strategic planning to meet present and future needs.
- Serve as grievance official and keep a log of resident complaints and resolutions.
- Supervise the Activities Coordinator in the performance of their duties and ensure that Special Olympics and campus events are successful.
- Participate in an on-call rotation schedule.
- Other duties as assigned.
Qualifications
PREFERENCES:
Preference may be given to applicants credentialed as a Certified Social Worker (CSW) or a Licensed Clinical Social Worker (LCSW).
KNOWLEDGE:- Knowledge of agency department and statewide rules regulations policies and procedures.
- Knowledge of Skilled Nursing Facility regulations licensing and survey processes.
- Knowledge of admission discharge and transfer criteria and practices in a SNF.
- Knowledge of best practices in the field regarding person-centered planning and provision of support services.
- Knowledge of the requirements for the Life Safety Code Department of Health and WLRC rules policies and procedures. Knowledge of the State of Wyoming Personnel Rules.
- Knowledge of learning strategies activities and issues involved in all direct services to residents and the care planning processes.
- Skills in verbal and written communication and the ability to communicate in a positive and effective manner to clients staff guardians families the public and all stakeholders.
- Ability to provide leadership and supervise staff including motivation team-building and performance evaluations.
MINIMUM QUALIFICATIONS:Education:Bachelors Degree (typically in Social Services)Experience:2-4 years of progressive work experience (typically in Casework) with acquired knowledge at the level of a Casework Manager IOREducation & ExperienceSubstitution:5-7 years of progressive work experience (typically in Casework) with acquired knowledge at the level of a Casework Manager ICertificates Licenses Registrations:NoneNecessary Special Requirements
PHYSICAL WORKING CONDITIONS:- Ability to work alone and/or with multiple residents.
- Ability to monitor sounds location of residents etc. in order to assure safety and assess imminent danger and emerging concerns.
- Must be able to safely move (twisting bending lifting kneeling pushing pulling) position and lift people with multiple and severe disabilities as necessary.
- Ability to work in high-stress situations.
- Ability to work around and with residents with aggressive and challenging behavioral issues. Ability to physically intervene during resident behavior episodes when needed.
- Work evenings nights and/or weekends when the situation requires it.
NOTES:
- FLSA: Exempt
- The Wyoming Life Resource Centerparticipates in E-Verify. - A background check including the taking of fingerprints will be completed through the Wyoming Department of Criminal Investigation Department of Family Services and the F.B.I. for every successful applicant.
