GENERAL DESCRIPTION:


This position serves as a Social Worker for the Skilled Nursing Facility (SNF) at the Wyoming Life Resource Center (WLRC) in Lander. Team members enjoy an excellent compensation and benefits package!

ESSENTIAL FUNCTIONS: The listed functions are illustrative only and are not intended to describe every function which may be performed at the job level.


  • Work with the Interdisciplinary Team and Administration to promote and protect resident rights and each residents psychosocial well-being.
  • Work with resident families and/or guardians to provide support information and education.
  • Work with the Admission/Discharge Coordinator to facilitate SNF admissions discharges and transfers to ensure all documents and plans are in place in accordance with federal and state regulations.
  • Clinically assess the residents mental health and complete assessments that identify social emotional and psychological needs.
  • Participate in the resident care planning process in developing an interdisciplinary plan of care for each resident.
  • Monitor and ensure that resident psychosocial needs are being met including treatment by staff resident-to-resident interactions and opportunities for community activities.
  • Work with all stakeholders such as the Wyoming Department of Health Behavioral Health Division Wyoming State Hospital Aging Services Division and community nursing home providers to determine eligibility and proper placement for persons served.
  • Participate in strategic planning to meet present and future needs.
  • Serve as grievance official and keep a log of resident complaints and resolutions.
  • Supervise the Activities Coordinator in the performance of their duties and ensure that Special Olympics and campus events are successful.
  • Participate in an on-call rotation schedule.
  • Other duties as assigned.

Qualifications


PREFERENCES:


Preference may be given to applicants credentialed as a Certified Social Worker (CSW) or a Licensed Clinical Social Worker (LCSW).


KNOWLEDGE:




Education:
Bachelors Degree (typically in Social Services)
Experience:
2-4 years of progressive work experience (typically in Casework) with acquired knowledge at the level of a Casework Manager I
OR
Education & ExperienceSubstitution:
5-7 years of progressive work experience (typically in Casework) with acquired knowledge at the level of a Casework Manager I

NOTES:

  • FLSA: Exempt
  • The Wyoming Life Resource Centerparticipates in E-Verify. We will provide the Social Security Administration and if necessary the Department of Homeland Security with information from each new employees Form I-9 to confirm work authorization. Please note that we do not use this information to pre-screen job applicants.
  • A background check including the taking of fingerprints will be completed through the Wyoming Department of Criminal Investigation Department of Family Services and the F.B.I. for every successful applicant.
  • Pre-employment drug and alcohol testing will be conducted on safety-sensitive positions. Drug testing will be conducted on any position if reasonable suspicion exists.
