Job Description

Under the direction of the Manager Clinical Services and in collaboration with Homeless and Housing Help Hub (H4) Staff within a multidisciplinary team the Case Manager Shelter Health will provide safe effective and ethical client care. The incumbent will provide direct professional nursing care to H4 clients requiring primary and mental health services including those clients who have been placed on a Community Treatment Order (CTO) by their psychiatrist or physician. The incumbent is responsible for assertive outreach to individuals within the Shelters. The incumbent provides professional case management services within the framework of a client directed clinical/rehabilitation case management model and according to the specifications of the CTO for those clients on a CTO. The incumbent provides a continuum of functions including assertive outreach building therapeutic relationships developing support plans with clients establishing community linkages client and systems advocacy symptom management life skills teaching supportive counseling family support crisis intervention medication monitoring and administration of injections. They will be responsible to communicate observations and concerns about CTO clients to their psychiatrists and other medical concerns to their physician.

RESPONSIBILITIES & DUTIES:

In collaboration with the Homeless and Housing Help Hub Staff the position responsibilities include the following within the scope of practice guidelines provided by the College of Nurses:

  • Assertive outreach to individuals in Shelters.
  • Establishing rapport and forming trusting relationships that provide the opportunity to assess needs and gain mutual agreement with the client on a treatment plan.
  • Working with a multi-agency team at point of care improving case planning by assessing needs and gaining agreement on a plan of care.
  • Improving access to care by conducting case planning promoting linkages to appropriate resources and by providing education and assistance interfacing with the health care and social service systems.
  • Monitoring clients while in the Shelter care to ensure safety.
  • Under authorization of a psychiatrist or physician receives medication orders administers medications including injectable medications where appropriate. Client injections include long-acting psychotropic medication vitamin B12 MOHLTC funded vaccines.
  • Communicable Disease testing and follow up health assessments.
  • Management of medication including receipt and storage of medications.
  • Uses a high level of nursing knowledge critical thinking skills sensitivity and good judgement when assessing clients and circumstances for the purpose of treatment and support.
  • Provides clinical intervention to support client needs.
  • Provide support to clients with concurrent disorders.
  • Develops goal focused support/care plans based on clinical assessment and reassessment with client and other supports as appropriate and requested by client including family members substitute decision makers public guardian/trustees and/or other natural supports service providers.
  • Provides clinical nursing assessments regarding medications side effects and medical conditions to care providers where required within the circle of care
  • Conducts ongoing assessment and observation for effects and side effects to the prescribed medication. Acting with client consent as liaison with clients psychiatrist/physician. Effectively communicates assessments and concerns regularly with team members and psychiatrists/physicians and collaborates to establish interventions and supports based on client needs and goals.
  • Maintenance and quality assurance related to medical equipment and point of care testing.
  • Client education e.g. glucometer teaching smoking cessation healthy lifestyles.
  • Adhering to standards as set out by the College of Nurses of Ontario and agency Policy and Procedures.
  • Following agency procedures and College of Nurses of Ontario Standards regarding documentation of medication orders medication administration and reporting of errors.
  • Keeping abreast of other resources in the community; Maintaining professional contact with commonly accessed organizations so that purposeful interaction on behalf of clients can be effectively accomplished.
  • Reducing stigma by actively working towards overcoming barriers to community integration
  • Participate in program development and evaluation; ensure program objectives are met
  • Ensures all file management and reporting functions are maintained in a comprehensive and timely manner
  • Other duties as assigned and which are within the scope of the position and which further the objectives/goals of the organization.

EDUCATION & SKILLS:

  • Requires knowledge and skills acquired through completion of university degree. (.N) in Nursing. Current registration with the College of Nurses of Ontario is required.
  • Two years recent relevant clinical work in the judgment of the employer with vulnerable individuals.
  • Comprehensive knowledge of mental illness and treatments; comprehensive knowledge and skill in the functions and principles of case management; up to date knowledge of relevant mental health policy and legislation including the Mental Health Act; knowledge of community resources; effective oral and written communication skills.
  • Computer literacy skills and working knowledge or various Microsoft Windows application is required.
  • Bilingualism in both official languages at the advanced level is considered an asset.
  • A valid Ontario Drivers License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.

Hours of Work: Thirty-Five (35) hour work week. The agencys regular hours of operation are Monday to Friday 4:00PM AM to 12:00AM. Flexible hours are required to meet client and service needs.

Salary range: Grade 9 $37.84 $44.81 per hour

Per 2022 - 2026 Collective Agreement

Applicable shift premiums are paid in accordance with the Collective Agreement Article 17.11.
