Job Description
- Full Time Permanent Contract
- 80 Hours per fortnight - Inclusive of ADO accrual
Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute sub-acute aged care and community services.
The Complex Care Service is seeking a motivated and team-focused Registered Nurse or Allied Health Professional to join the team as a Care Coordinator providing intensive time limited intervention to clients with chronic health conditions and/or complex health needs. The role of the Care Coordinator is to provide comprehensive clinical assessment client education and support to identify barriers to self-management make referrals to services and to coordinate the clients care as part of a multidisciplinary team.
You would be joining a dynamic and supportive team of highly experienced professionals and providing care in clients homes and via telehealth. You will receive comprehensive orientation to the role as well as clinical supervision and professional development opportunities.
