The role is Monday-Friday 40 hours per week with a highly-flexible schedule. No weekends or holidays.
Opportunity Summary:
Origami Rehabilitation is seeking a full-time (40 hours per week Monday through Friday) compassionate and detail-oriented Residential Care Coordinator (RCC) to support our clients through thoughtful coordination and communication of care.Working under the guidance of the Clinical Nurse and Residential Manager youll collaborate with an interdisciplinary team to ensure every resident receives the individualized holistic care they deserve.
In this role youll be a steady and trusted presence keeping care plans on track facilitating effective communication between stakeholders and advocating for residents in a way that aligns with Origamis mission vision and values.

While performing the duties of this job the employee is regularly required to talk and hear. The employee is occasionally required to stand walk sit use hands to finger handle or feel reach with hands and arms climb or balance stoop kneel or crawl and taste or smell. Specific vision abilities for this position include close vision distance peripheral vision depth perception and ability to adjust focus.
