Position Summary
Under the direction of the Associate Dean of Students and Director of Engagement and Support Programs the Clinical Care Coordinator provides intervention and case management services designed to promote individual student success and retention. This is accomplished through effective case management and coordination of care between campus and community resources. The Clinical Care Coordinator conducts intervention and case management for students experiencing academic behavioral medical mental health and other personal concerns. The Clinical Care Coordinator serves as a member of the Student Support Network (behavioral intervention team) and supports an ongoing caseload of students who benefit from a systems approach to student success and retention. Duties include triage assessment intervention crisis management and support/care plan development. The Clinical Care Coordinator conducts outreach education follow-up and communication with campus stakeholders including faculty staff students and family members. The Clinical Care Coordinator engages in ongoing needs assessment intervention documentation and tracking.
Physical Demands
Extensive use of computer equipment. Work in an office environment that is very busy and with constant interruptions frequent phone calls and walk-in traffic. Continuous sitting; frequent lifting/carrying up to 15 lbs of materials (such as notebooks books and paper).
Duties
Related Other Duties Training and Outreach: Provide individual and group outreach and support for historically underserved populations and veterans reentry students students in recovery and students formerly in foster care. Other duties as assigned
Preferred Qualifications
Experience in resource coordination and case management of college students. Previous work on a college or university behavioral intervention team.
