Job Description

Join a team where compassion meets purpose. Community Health Services (CHS) is seeking a dedicated Nurse Practitioner (APRN) to serve in our Adult Medicine Department delivering high-quality primary care to a diverse patient population in the heart of Hartford.

This is more than a clinic role it s a chance to make a lasting impact in the community while working alongside a supportive multidisciplinary team.

What You ll Do

  • Provide comprehensive primary care to adult patients focusing on prevention wellness and chronic disease management.

  • Collaborate with physicians and specialists to ensure coordinated patient-centered care.

  • Conduct assessments make evidence-based clinical decisions and document care using our electronic health record.

  • Offer culturally competent care to a diverse population including underserved and at-risk groups.

  • Support department goals for quality safety and patient experience.




Requirements



  • Completion of an accredited nurse practitioner program

  • Current licensure (or eligibility) in the state of Connecticut

  • DEA and state narcotic registration

  • 1 2 years experience in ambulatory or urgent care preferred

  • Excellent communication and clinical decision-making skills

  • Comfort working with EHR and care protocols

  • Spanish-speaking candidates are strongly encouraged to apply

  • Monday Friday 8:30 AM 5:30 PM (flexible part-time options available)

  • Nine-hour shifts include one hour for administrative tasks and one for lunch

  • Goal: 14 patients per day

  • Team-oriented environment with 45 50 department staff and growing

What You'll Need to Succeed A valid Physical Therapist license in the state of California. CPR certification that s up to date. A current California driver's license, a clean driving record, and dependable transportation for home visits. Availability for a minimum of 25 patient visits per week to be considered full-time. At least two weekdays per week of availability, and for PTAs, two weekend days per month. Bonus Points If You Have: Prior experience in a home health setting (not required, but preferred). A strong commitment to patient-centered care and collaborative teamwork.
