Medford OR Flexible Schedule Patient-Focused Care
Are you a dedicated Physical Therapist looking to make a meaningful impact in patients lives right in the comfort of their own homes We re seeking a compassionate and skilled professional to join our Home Health division where you ll provide high-quality individualized care to patients recovering from illness injury or surgery.
Key Responsibilities
Conduct thorough physical assessments and review patient medical histories
Develop and implement customized treatment plans based on individual needs and goals
Deliver therapeutic exercises and training to promote recovery and independence
Educate patients and families on injury prevention wellness and care techniques
Monitor and document progress toward rehabilitation goals
Collaborate with interdisciplinary healthcare teams for well-rounded patient support
Maintain timely accurate records in compliance with regulatory standards
Requirements
Master s degree in Physical Therapy
Minimum of 1 year of home health experience as a physical therapist
Current state licensure to practice as a physical therapist
Active CPR certification (must be maintained during employment)
Valid driver s license and reliable transportation for travel to patient locations

What You'll Need to Succeed A valid Physical Therapist license in the state of California. CPR certification that s up to date. A current California driver's license, a clean driving record, and dependable transportation for home visits. Availability for a minimum of 25 patient visits per week to be considered full-time. At least two weekdays per week of availability, and for PTAs, two weekend days per month. Bonus Points If You Have: Prior experience in a home health setting (not required, but preferred). A strong commitment to patient-centered care and collaborative teamwork.
