Location: Orlando FL
Guaranteed Hours: (40 hours/week)

Job Description:
We are seeking a skilled and compassionate Home Health Physical Therapist to provide high-quality rehabilitation services to patients in the comfort of their homes. The therapist will evaluate patient needs develop care plans and help patients restore mobility strength and independence.
Key Responsibilities:
- Conduct initial patient assessments and evaluations.
- Develop and implement individualized treatment plans.
- Provide physical therapy interventions to improve mobility function and pain management.
- Educate patients and caregivers on exercises home safety and equipment use.
- Document patient progress treatments and outcomes accurately and timely.
- Collaborate with physicians nurses and the interdisciplinary care team.
- Ensure compliance with all home health policies procedures and regulatory standards.
Required Qualifications:
- Doctor of Physical Therapy (DPT) or Masters in Physical Therapy (required).
- Active Florida Physical Therapist (PT) License (required).
- Minimum 1 year of physical therapy experience (home health experience).
- Strong clinical interpersonal and communication skills.
- Current CPR/BLS certification.
