Job Description
SUMMARY: The Behavioral Health Case Manager (BHCM) coordinates services for individuals including those with complex needs to ensure they receive holistic care access appropriate resources and progress towards their act as an advocate and facilitator working with clients and their support systems to develop and implement service plans monitor progress and address any emerging challenges. BHCM will make referrals to other community resources for services not provided by the NACCs Behavioral Health Program. This individual will participate in team meetings and supervision as . They will follow clinic policies and procedures in all aspects of the position and participate in educational trainings and workshops for professional growth and development.
DUTIES AND RESPONSIBILITIES:
- Meet with newclients referred toBHCMtoassessappropriatenessfor BHCMservices via scheduled and walk-in appointments.
- Complete a BHCM intake with clients who are determined to be appropriate and work with providers and other care team members to identify needs and establish next steps for care.
- Meet with clients to develop a case management plan that prioritizes the clients goals and desires.
- Provide care coordination to BHCM clients from aharm reduction trauma informed and strength-based approach.Workto empower clients in their recovery process and to meet their goals.
- Collaborate with providers to arrange for external and internal referrals as deemed necessary.
- Partner with other NACC care coordinators and support staff to meet the needs of clients and share resources.
- Attend case consultations with medical and behavioral/chemicalhealth providers to provide updates and obtain next steps in care coordination.
- Obtain releases of information to ensure regular communication with other providers about ongoing care.
- Assist clients in crisis with access to timely assessment intervention and stabilization services.
- Engage high risk clients and when appropriate their family members to ensure understanding of and support with individual care plan.
- Document all service provision and consultation in clients electronic medical record in a timely manner.
- Monitor medical and behavioral health clinic schedules for opportunties to engage clients in care coordination via walk-ins/same days.
- Assist with administering and tracking/monitoring screenings and patient satisfaction measures. Report on performance improvement and quality measures related to population-based care coordination.
- Participate in meetings and work groups for quality care coordination and staff training and development.
- Help to assess and evaluate the effectiveness of services received by clients such as reviewing client service utilization data assisting in the collection of client satisfaction surveys and creating and collecting data on service specific client surveys.
- Assist with marketing tasks for BHCM.
SUPERVISORY RESPONSIBILITIES:
- This job has no supervisory responsibilities.
QUALIFICATIONS:
Must meet qualifications for a Mental Health Practitioner in Minnesota:
- Bachelors Degree in social work psychology or other relevant field.
- OR the qualified health home specialist must meet one of the following qualifications:
- A peer support specialist
- A family peer support specialist
- A case management associate
- A mental health rehabilitation worker
- A community paramedic
- A peer recovery specialist
- A community health worker
- Experience care coordinating complex patient needs.
- Experience working with Native American population preferred.
