Role Summary:
The Clinical Pharmacist assists in the development and execution of numerous initiatives that improve the quality or cost effectiveness of drug therapy in coordination with other care team members for Evernorth Care Group. This role supports Diabetes Medication Management Anticoagulation Management Transitions of Care Medication Reviews.
Responsibilities:
Perform as a Clinical Pharmacist for the assigned healthcare centers
Assist in the development support and monitoring of disease management and drug utilization management programs
Identify classes of drug therapy with potential for improvement in quality of care increased efficiency and improved cost-effectiveness
Identify Value Based Care gaps and collaborate with providers to improve quality metrics.
Responsible for development and monitoring of internal detailing process
Develop and present education to providers patients and clinic staff through newsletters presentations/in-services and one-on-one contact
Collaborate with providers to discuss quality of care cost effectiveness and utilization management
Serve as a clinical pharmacy resource for all clinic staff
Qualifications:
Bachelors Degree in Pharmacy required; PharmD preferred
Pharmacy License active in State of Arizona required
Minimum of 5 years of experience in Clinical Pharmacy or completion of a Clinical Pharmacy Residency program with a minimum of one year of experience in a managed care setting
Excellent verbal communication skills with the ability to communicate and maintain effective interpersonal and working relationships with professional and support staff providers and all customers
Excellent analytical and problem-solving skills
Ability to effectively educate Providers and staff members
Proficient with Microsoft applications including Outlook Word Excel and PowerPoint
Additional credentials such as BCACP BCPS CACP CDCES and/or BC-ADM preferred
Previous experience/training inmedication reconciliation and MTM services
Bilingual skills (English/Spanish)
Previous experience with electronic health records
