Job Description
Position Title: Clinical - LTSS Service Care Manager
- LVN License
- Assist in telehealth biannual assessments in person as needed if in the area.
- Complete assigned tasks and document findings.
- Coordination with multi-disciplinary teams.
- Follow up calls to members.
- Any other projects that may arise.
- Must be able to meet strict turn-around times with documentation for assessments assigned.
- Microsoft Word Excel experience Computer literate
- At lease 2 years experience at LVN Home Health experience Case management experience
