Job Summary:
This role provides high-quality occupational therapy within an integrated health and social care setting supporting adults and older people with complex needs including those within the prison cluster.
The position requires a person-centred approach to enhance individuals independence safety and quality of life through assessment rehabilitation equipment provision and environmental adaptation.
The postholder will also contribute to service planning performance management and the supervision and development of junior staff.
Key Duties/Accountabilities (Sample):
Conduct complex assessments and provide interventions that promote independence and improve outcomes for vulnerable adults older people and children in transition to adult services.
Deliver occupational therapy services through the Care Management process including advice equipment prescription and home adaptations.
Undertake safeguarding assessments in line with statutory responsibilities and multi-agency procedures.
Provide specialist advice guidance and supervision to colleagues and students as delegated by line management.
Contribute to continuous service improvement policy development and multi-agency collaboration ensuring compliance with professional legal and ethical standards.
Liaise with internal and external agencies ensuring smooth care transitions timely service delivery and person-centred support.
Represent the service in professional meetings panels and forums and lead on service development areas where required.
Maintain accurate timely case records and reports in accordance with data protection and professional guidelines.
Skills/Experience:
Proven experience working with adults and older people in a complex social care environment ideally including prison hospital discharge or safeguarding settings.
In-depth knowledge of the Care Act 2014 safeguarding legislation housing and social care frameworks and occupational therapy best practice.
Skilled in conducting holistic assessments risk management and implementing interventions using a strengths-based approach.
Ability to manage and prioritise a diverse caseload including high-risk and urgent cases.
Experience in supervision service planning and performance monitoring.
Competence in using a range of IT systems and recording tools for case management and reporting.
Strong interpersonal and communication skills with a commitment to promoting equality diversity and inclusive practice.
Additional Information:
Professional qualification in Occupational Therapy and current registration with the Health and Care Professions Council (HCPC).
