Job Description

THE POSITION

DESCRIPTION OF WORK

An Aging Care Manager 2 provides consumers family and caregivers with information and directions related to the process of obtaining Waiver Services for the community nursing facility (NF) placement and/or personal care home (PCH) placement. Work involves completing a Functional Eligibility Determination (FED) assessment to determine eligibility and appropriate level of care for all ages of consumers residing in Lebanon County. This includes nursing facilities individuals homes hospitals or personal care homes. Collaboration with the supervisor will ensure the consumer is appropriately placed for their current level of need. Key to this position will be linking consumers family member and caregivers to available community services and resources in Lebanon County to provide continuity of care.

Other duties to include but not limited to:

  • Completing regularly assigned desk reviews
  • Completing assigned assessments accurately and in a timely manner
  • Attending appeals and hearings as needed
  • Attending agency and inter-agency meetings to review policies and procedures relating to AAA and other agencies within the community


Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:30 PM Monday - Friday with a 30-minute lunch.
  • Telework:You will not have the option to telework in this position.
  • Salary:In some cases the starting salary may be non-negotiable.

REQUIRED EXPERIENCE TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months as an Aging Care Manager 1 or a County Caseworker 1;or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelors degree in social sciences behavioral sciences human services or a closely related field; or
  • An equivalent combination of experience and training.
