**Job Title: Home Health Registered Nurse (RN)**

**Salary:** $55000-$75000 per year
**Job Summary:**
We are seeking a compassionate and dedicated Home Health Registered Nurse (RN) to join our team. The ideal candidate will possess clinical expertise and a strong commitment to delivering quality patient care in a home setting. As a Home Health RN you will play a vital role in assessing planning and implementing nursing care for patients in their own homes ensuring they receive the support they need to maintain their health and independence.
**Key Responsibilities:**
- Conduct comprehensive patient assessments to develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient vital signs progress and overall health status providing necessary interventions.
- Collaborate with physicians caregivers and other healthcare professionals to coordinate care.
- Educate patients and their families on health conditions medications and home care techniques.
- Document all patient interactions and changes in condition accurately and in a timely manner.
- Ensure compliance with all state and federal regulations and maintain patient confidentiality.
- Assist patients with activities of daily living (ADLs) as needed.
- Participate in team meetings and continuing education opportunities to improve care quality.
- Provide emotional support and counseling for patients and families in navigating health challenges.
**Qualifications:**
- Current Registered Nurse (RN) license in Texas.
- Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
- Minimum 2-3 years of nursing experience with at least 1 year in home health or community health setting preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS) certification preferred.
- Proficient in the use of electronic health records (EHR) and other relevant technology.
- Valid drivers license and reliable transportation to travel to patients homes.
**Physical Demands:**
- Ability to stand walk and lift up to 50 pounds as needed.
- Capability to perform tasks requiring physical endurance including bending kneeling and reaching.
