Job Description:

• Conduct thorough and ongoing assessments of pre- and post-transplant patients including physical psychological and social needs.
• Develop implement and evaluate individualized nursing care plans in collaboration with the multidisciplinary transplant team (physicians surgeons social workers dietitians etc.).
• Monitor patient vital signs laboratory results and clinical status recognizing and reporting any deviations or complications promptly.
• Administer and monitor immunosuppressive medications ensuring adherence to prescribed regimens and recognizing potential side effects.
• Provide specialized care for transplant patients including managing lines drains and surgical sites.
• Recognize and respond to signs and symptoms of organ rejection infection and other post-transplant complications.
• Assist with pre-transplant workups and evaluations including patient preparation and coordination of tests.
• Participate in the organ allocation process and preparation for transplant surgery.
• Educate patients and their families comprehensively on transplant procedures medication regimens lifestyle modifications and signs/symptoms to report.
• Provide emotional support and counseling to patients and families coping with the complexities of transplant.
• Reinforce the importance of medication adherence follow-up appointments and long-term health management.
• Adhere to all hospital policies procedures and regulatory requirements including infection control and patient safety protocols.
• Participate in quality improvement initiatives related to transplant care.
• Maintain a safe and therapeutic environment for patients.
