3 months contract with local authority
We are seeking a dedicated individual to join our Islington Reablement Service team providing intensive short-term support to enable individuals to regain or maximize their independence. As an Enabler you will work with adults with a range of needs including learning disabilities physical and sensory needs mental health and older people across various community and hospital settings.
Key Responsibilities:
- Delivering personalized care and support in accordance with individuals Reablement plans
- Working closely with health and social care professionals to ensure effective care planning and delivery
- Demonstrating best practice in strengths-based and person-centered approaches
- Collaborating with residents carers and community-based organizations to promote independence
- Adhering to the Councils policies and procedures including safeguarding and the Care Act 2014
- Providing support with medication nutritional needs and specific dietary requirements
- Maintaining accurate records and documentation in compliance with organizational standards
- Ensuring infection control and legal regulatory and policy compliance
- Participating in multi-disciplinary meetings and contributing to service outcomes and personal development
Requirements
- Willingness and ability to travel across the teams geographical area
- Flexibility to work over a 7-day working week including morning evening and overnight shifts
- Commitment to public service quality equality and empowerment

- Compliance with GDPR Health and Safety and other relevant regulations

- Ability to undertake other duties commensurate with the grade of the post
- From April 2022 full vaccination in line with the Governments Coronavirus (COVID-19) guidelines for the health and wider social care sector
Requirements: Proven experience in quality assurance and reviewing services for children and young people Strong understanding of statutory and regulatory safeguarding requirements Excellent communication and interpersonal skills Ability to work effectively in a fast-paced environment Commitment to championing the rights and well-being of children and young people
