The Care Manager-RN role is integral in enhancing patient outcomes by managing complex health issues through thorough assessments developing detailed discharge plans and facilitating treatment across various settings. This position requires close partnership with doctors nurses patients and families to optimize health service delivery and ensuring all medical psychological and social needs are met efficiently. The Care Manager is responsible for Utilization Review ensuring that medical treatments provided are necessary and within the best cost parameters while adhering to quality standards. Collaboration with billing offices and insurance providers is essential to address coverage issues and secure patient benefits. Typical responsibilities include ongoing patient monitoring advocating during care transitions and contributing to team-led patient support initiatives.
In Hospital Care Management at CRMC we ensure that patients receive the right care at the right time in the right settingeven after they leave our hospital. From coordinating discharge plans and arranging follow-up care to addressing social determinants of health our team ensures a smooth transition from hospital to home rehab or other care facilities. The pay range listed above is the estimated starting pay range for this role. Actual pay rate offered depends on many factors including each individual employees experience level preferred education/certifications along with market conditions.
