The Patient Navigator is a Licensed Vocational Nurse with recognized leadership abilities and sound clinical skills who will actively assist the physician in the day to day activities and care of the patient. This role will also include arrangement of the details of patient care in the units service line. The LVN Patient Navigator takes a leading role in activities in the unit patient rounding and care identifying and assessing patient needs scheduling and working with the Physicians RN coordinator employees and preceptors in the orientation and training of new employees. Working in close collaboration with the RNs and M.D.s preparing teaching and guiding patients through the continuum of care. Executes an expertise patient care Support of Systems Research Education and Professional Leadership. He/she works collaboratively with the nursing staff medical staff educators managers and all members of the health care team to accomplish the responsibilities outlined below.
