Location: Tuba City AZ 86045
Assignment Start Date: 8/11/2025
13 weeks Days 8-hour shift 40 hours per week

Please get in touch with a recruiter for full details.The Physical Therapist I/II / Advanced Physical Therapist I/II position serves to provide quality physical therapy services to a predominantly Native American population for the treatment or prevention of physical disability or disease. Responsible for management and implementation of Physical Therapy program; provision of physical therapy evaluation and treatment of inpatients (regular work week and weekend rotation) and outpatients ages ranging from birth through geriatric; provision of specialty services such as wound care prosthetic and orthotic clinic geriatric clinic high school clinic functional clinic spinal cord clinic assistive technology/adaptive equipment evaluation/treatment Durable Medical Equipment Prosthetic Orthotic Supply Program; assumes direction and development of the physical therapy section to include continuing quality improvement risk management and safety physical therapy policies and procedures; Joint Commission preparation staff education reporting staff meeting and in-service and other duties. Responsibilities also include those listed on the Medical Staff Credentialing Core Privileges form.
EXPERIENCE/EDUCATION/QUALIFICATION
Experience:
- Must have at least 3 years of previous experience
- Applicant must meet requirements to be credentialed to serve on the Medical Staff.
Education:
- Must have a Bachelors degree from a Commission of Accreditation in Physical Therapy Education (CAPTE) accredited school of Physical Therapy.
Licenses Certifications and/or Registration:
- Must possess a valid current full and unrestricted license in any state within the United States the District of Columbia or the Commonwealth of Puerto Rico during tenure of employment.
- Must have and maintain a current Basic Life Support (BLS/BCLS) certification by the American Heart Association.
EMR System: Allscripts experience preferred
Shift: Monday-Friday 8 am-5 pm
Orientation: Every two (2) weeks only.

Housing Application: must be submitted 2 weeks before the start date
