POSITION SUMMARY

The Kids Come First Health Team is a provincial leader in mobilizing partners to develop and implement innovative services and programs to meet the needs of children and youth in Eastern Ontario. Our team includes over 60 hospital- and community-based organizations; youth and family/caregiver partners nearly 1100 physicians; and many other individuals. Our vision is to provide high-quality connected care for children and youth and simplify their healthcare journey across the health and social system.

CHEO is the host organization for the Kids Come First Health Team.

Reporting to the Manager the successful candidate will primarily work on a hospital-community collaboration to improve trauma-informed and culturally sensitive alcohol and substance use screening brief intervention referrals and treatment for youth. The successful candidate will work on an interdisciplinary team to coordinate client care related activities and provide leadership and education to designated staff. This may include planning patient discharge upon the onset of admission; liaising with the community; facilitating transfers to other facilities; and supporting youth and their families and caregivers as they transition back to the community. Strong knowledge and expertise in substance use health and concurrent disorders is critical.

Main Responsibilities (This is not an exhaustive list):

Characteristic responsibilities include:

  • Coordinate client care related activities according to criteria care paths and processes in collaboration with a multidisciplinary team.
  • Prepare and facilitate case planning meetings including determining/Inviting appropriate members
  • Promote collaboration between service providers to develop flexible and creative client service plans
  • Review client data to determine eligibility for referral and collaborate with referring agencies to work towards resolution and ensure the most efficient/appropriate referral process is accessed
  • Provide discharge planning and coordinate services both within the hospital and in the community.
  • Participate in interdisciplinary team rounds providing knowledge and expertise in the treatment planning of the psychological social and emotional needs of youth.
  • Maintain awareness of developments and changes to services and programs in the hospital and community settings
  • Provide ongoing assessments of psychosocial factors that could impact on the clients well-being
  • Assess family dynamics functioning strengths and weaknesses
  • Develop and implement services and programs to respond to the psychosocial needs of the client parent family and/or caregiver
  • Provide emotional support/counseling (e.g. trauma crisis intervention adjustment behavior modification grief/bereavement etc.)
  • Identify advocate for negotiate and obtain community resources pertinent to the needs of the client and their families and caregivers
  • Maintain psychosocial documentation current with accurate and pertinent information of the client and family situation
  • Complete daily statistical documentation
  • Education and Training:
  • Facilitate the identification and development of clinical staff learning needs through education coaching and mentoring
  • Act as a resource to staff to promote best clinical practices assess clinical skills and liaise with other healthcare professional leaders
  • Provides leadership in the education of allied health professionals volunteers and students
  • Support quality patient care and excellence in practice as it relates to mental health addictions substance use health and concurrent disorders
  • Supervise and train social work students at the BSW and MSW levels and provide practicum opportunities.
  • Participate in panel discussions and educational rounds
  • Perform work in accordance with the provisions of the Occupational Health and Safety Act and Regulations and all CHEO corporate/program policies and procedures related to Occupational Health and Safety.
  • Participate in various discipline and interdisciplinary committees throughout the hospital (e.g. Social Work Family Forum PLEO Community Advisory Committee) as required
  • Participate in Quality Management Policy and Planning and Accreditation Activities as required
  • Consult and collaborate with a variety of community agencies (e.g. ministries school boards child welfare authorities police) as required
  • Perform other related duties as assigned by the manager/director

QUALIFICATIONS

  • Superior leadership interpersonal and oral and written communication skills(Essential)
  • Proven analytical and problem-solving skills including advanced clinical assessment skills(Essential)
  • Effective organizational and administrative skills to coordinate patient care activities in collaboration with a multidisciplinary team(Essential)
  • Demonstrated desire to promote excellence in culturally sensitive trauma-informed and patient-centered care
  • Knowledge of mental health addictions and substance use health resources and programming in the hospital and community setting (Essential)
  • Knowledge of child protection issues(Essential)
  • Knowledge of applicable occupational health and safety legislation and general knowledge of corporate/program policies and procedures for health and safety(Essential)
  • M.S.W. graduate from an accredited university program in social work (Preferred)
  • B.S.W. graduate will be considered if other qualifications are met (Essential)
  • Membership in the Ontario College of Social Workers and Social Service Workers in good standing(Essential)
  • Minimum three (3) years of work experience in relevant health or social discipline (Essential)
  • Strong written and verbal proficiency in English and French (Essential)
  • Clinical proficiency and expertise in addictions and/or substance use health(Essential)
  • Experience in knowledge translation and staff teaching coaching and mentoring (Essential)
  • Police record check (Essential)
  • Take steps to ensure timely completion of tasks by adjusting priorities as required(Essential)
  • Deal effectively and sensitively with patients and/or relatives to obtain cooperation and approval of actions(Essential)
  • Deal effectively in crisis or traumatic situations involving extreme emotional situations(Essential)
  • Deal effectively with external customers of considerable importance(Essential)
  • Functions well in an interdisciplinary team.(Essential)

La personne retenue relvera de la ou du gestionnaire de et travaillera principalement dans le cadre dune collaboration entre lhpital et la communaut pour amliorer le dpistage des troubles lis la consommation dalcool et de substances les interventions de courte dure les aiguillages et les traitements tenant compte des traumatismes et des diffrences culturelles pour les jeunes. La personne retenue travaillera avec une quipe interdisciplinaire pour coordonner les activits lies aux soins des clientes et clients et assurera un leadership et offrira de la formation au personnel dsign. Il peut sagir de planifier le cong de la patiente ou du patient ds ladmission dassurer la liaison avec la communaut de faciliter les transferts vers dautres tablissements et de soutenir les jeunes leur famille et leurs aidantes et aidants durant leur retour dans la communaut.
