Job Description

Public Health Nurse (4 Temporary full-time Positions)

The Middlesex-London Health Unit has an exciting opportunity for 3 temporary full-time Public Health Nurses to work until July 2026 and 1 temporary full-time Public Health Nurse to work until December 2026 on the iHEAL program on the Nurse Family Partnership and Early Years Group Programs (NFP&EYGP) Team within the Family and Community Health division at our Citi Plaza location London Ontario.

iHEAL is an evidence-based health promotion program designed to improve the safety health and well-being of women who have experienced intimate partner violence.It is delivered by Registered Nurses who receive specialized training and work with women over a period of 6 to 7 months. Using a woman-led approach the program addresses each womans priorities in collaboration with existing community services and the iHEAL nursing team. MLHU is one of the first organizations in Canada to offer this novel program.

Key Responsibilities: Under the direction of the Associate Manager NFP&EYGP the incumbent

will:

  • Complete all aspects of the iHEAL program education requirements
  • Offer the iHEAL program providing woman-led personalized care according to the iHEAL program principles components and protocols
  • Conduct relevant health social and safety assessments
  • Support women to act on their priorities related to their health safety and well-being through coaching education modelling referral advocacy and related modalities
  • Work collaboratively with community agencies to support womens access to and engagement with supports and services as they desire
  • Engage in relevant external collaborative initiatives
  • Promote the iHEAL program with relevant community partners to establish and maintain appropriate referral pathways to/from the program
  • Maintain documentation as per agency guidelines utilizing the electronic client record
  • Contribute to Quality Improvement and iHEAL program reporting using relevant processes systems and documents
  • Participate in inter-team/interdisciplinary work within the Division
  • Perform other duties as assigned

QUALIFICATIONS:

  • Baccalaureate Degree from an accredited nursing program (BScN)
  • Current certificate of competence from the College of Nurses of Ontario (CNO)
  • Excellent interpersonal written/oral communication and facilitation skills
  • Proficient technical skills in the areas of documentation Microsoft Office & database software
  • Minimum 1 year experience working in community-based practice (e.g. public health primary care community mental health outreach services). Experience in community home-visiting is considered an asset.
  • Experience providing 1:1 nursing care in partnership with individuals experiencing or at risk for intimate partner violence and providing trauma and violence-informed care is required.
  • Knowledge of the complexities of intimate partner violence and the impacts on health and well-being
  • Knowledge of local services and experience in system navigation and engaging and working collaboratively with internal and community partners
  • Demonstrated expertise in critical thinking problem solving evidence-informed decision-making and navigating complex situations
  • Ability to work autonomously in a variety of community settings and contribute to an effective team environment
  • Knowledge and skills in relationship-building and creating a safe and inclusive environment with diverse populations
  • Knowledge and skills related to the social determinants of health and health equity with experience in assessing and addressing health inequities
  • Demonstrated commitment to contributing to a positive workplace environment
  • Communication proficiency (written and verbal) in French and/or any other language is considered an asset
  • Ability to work flexible hours
  • Valid Ontario Drivers license and own transportation

The hourly rate for this position ranges from $39.57 to $53.96 hourly based on qualifications and experience.
