What Youll Be Doing
You will assist the programs participants by building on their strengths using community-based support and connecting them with a wide range of assistance and resources. Program participants include children adults and families where clients have behavioral emotional and case management needs.
Hybrid Role 20% Remote Work.
Your Main Priorities
While additional responsibilities are likely to arise on the job you will primarily:
Utilize an in-home strengths-based team approach to provide a wide range of assistance and resources to support individuals and families.
Work with collateral contacts and key referral sources to coordinate care and support.
Document relevant client information in the agencys Electronic Health Record system.
Meet with Child & Family supervisor and team weekly and as needed to ensure quality care.
Work with clients to assess their situations needs and goals to determine the services required.
Implement individualized service plans developed with the clients and the team; examples include goals of connecting families to resources for basic needs and supporting improvement of communication and behavioral support among family members.
Provide support and guidance to individuals and families managing crises and be part of an on-call crisis support phone rotation.
Skills & Abilities
Strong organizational critical thinking and time-management skills.
Basic computer skills required; knowledge of Microsoft Office 365.
Ability to communicate and effectively interact with a diverse population.
Bilingual in Spanish preferred.
Collaboration and teamwork.
Must authorize Child & Family to complete a motor vehicle records check.
The ideal candidate will be required to meet all background checks and medical clearances required for the position.
Physical Requirements
Direct Service Staff:
This position requires time sitting standing walking driving carrying and lifting up to 25lbs. This role also requires operating desktop laptop and/or cell phone and communicating with colleagues.
Travel Requirements
Site location: Middletown or Providence
Statewide travel required.
