The Care Coordination Manager serves as an important liaison between facility-based employees and our clinical outreach team in improving communication with our strategic referent relationships. You will be working at a facility as part of a treatment team and in a business development/marketing role.
Essential Job Functions:
- Primary contact between our facility clinical outreach and referents in assuring communication and patient care coordination
- Attends appropriate clinical meetings for patient updates and discharge planning discussion
- Train and oversee Care Coordination team in support of our corporate initiatives regarding patient aftercare coordination and communication with our preferred providers
- Oversee referent communication with patient in our care and aftercare planning
- Establish positive working relationships with clinicians medical providers EAPs and Veteran Association Accounts
- Identify and overcome potential barriers to patient care
- Identify opportunities to refer patients in our care to preferred aftercare partners based on clinical outreach recommendations. Secure appropriate ROIs VOBs assessments and seamless transition for return to provider VA or step down to sober living outpatient programs and therapists/psychiatrists.
- Document and audit care coordination documentation in KIPU and salesforce
- Coordinate tours with referents and appropriate staff at facility
- Facilitate aftercare groups and individual meetings with patients regarding discharge planning
- Collaborate with family programming to provide resources to families
- Job may include additional responsibilities as needed for patient care
Core Competencies:
- Proficient in MS Office applications which include Outlook Word Excel and PowerPoint.
- Ability to build and sustain trusting relationships with diverse individuals and groups.
- Ability to maintain confidentiality of sensitive information and adhering to ethical standards.
- Actively offering assistance to team members when needed and celebrating team achievements collectively.
- Ability to establish and maintain cooperative relationships with community-based organizations patients families vendors and other resources to promote client services care education and advocacy.

Qualifications :
- Bachelors Degree or LCDC preferred
- 1-2 years experience in the mental health and/or chemical dependency
- Understanding of Addiction: Basic knowledge about substance use disorders
- Knowledge or ability to learn Salesforce KIPU or other EMR system
- Adapts to changing work environments work priorities and organizational needs
- Self-motivated and ability to work independently
- Identifying new referral sources and enhancing current relationship in collaboration with clinical outreach team
