The Hants Health and Wellness Team (HHWT) is a community based integrated chronic disease prevention and management service that provides wellness programming and navigation chronic disease management nutrition education and complex care. The HHWT is an interdisciplinary team that builds community connections and partnerships to provide programs and services that are responsive to community needs. Principles of patient-centred care behaviour change and self-management are core to all programming and services.
The Team Lead will report to the Health Service Manager Primary Health Care and will be responsible for development implementation and coordination of HHWT programs and service. This includes triage of referrals monitoring patient flow scheduling and supporting a variety of quality improvement and evaluation activities.
This individual acts as a role model and exhibits team building and leadership skills. S/he demonstrates effective communication and organizational skills works with an interdisciplinary team and collaborates with colleagues to determine team goals and objectives participates in the evaluation of team effectiveness solves problems and shares responsibilities. Excellent interpersonal and written communication skills are required.
We would love to hear from you if you have the following:
Baccalaureate degree in Human Nutrition Occupational Therapy Social Work Nursing or Physiotherapy
Registered member in good standing with a relevant health discipline college/association required
Current status as a Certified Diabetes Educator as required (or eligible within one year of hire)
Five years clinical experience required (with clinical experience in the last year working with individual and/or group patient education)
Previous experience in team development and leadership working within a collaborative interdisciplinary practice
Experience working in a Primary Health Care setting/community-based team required
Individual and group education/facilitation skills utilizing adult education self-management support and/or motivational enhancement principles
Experience in community development and working with people living with a chronic condition is preferred
Understanding of population health health promotion strategies chronic disease prevention and management and behaviour change principles is an asset
Problem solving ability and willingness to considered uncharted or novel ideas related to trends in healthcare
Coaching ability and diplomacy when dealing with co-workers clients and community members
Ability to establish and maintain good working relationships with clients colleagues management and community agencies
Required travel in the community
Valid Nova Scotia Drivers License and access to a reliable vehicle
Competencies in other languages an asset French preferred

$40.50 - $51.17 hourly
