The Diabetes Educator is involved with assessment diagnosis educating and proactively manage the patients condition. This position will be primarily at the South Family Medicine Clinic with every 7th weekend call rotation at the Medical Center.Summary
Educates patients on self-care involved in the diagnosis of diabetes; collects patient data to monitor health status and proactively manage the patients condition.Job Description
Organizes diabetes self-care materials including nutrition medication physical activity and glucose testing. Educates patients on the necessary lifestyle changes involved in the diagnosis of diabetes. Demonstrates familiarity with pathophysiology epidemiology and clinical practice consistent with practice level. Collects patient data on a regular basis in order to monitor their health status. Makes suggestions on how to improve health conditions based on the analysis of patient data; refers patients with specific problems to other healthcare providers. Applies principles of program and/or business management to create a climate that supports successful self-management of diabetes. Educates patients on self-care and lifestyle changes involved in the diagnosis of diabetes.
Qualifications
Must have a bachelors degree in nursing or willing to obtain within three years. Graduate from an accredited nursing program preferred including but not limited to American Association of Colleges of Nursing (AACN) Accreditation Commission for Education in Nursing (ACEN) and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) preferred.Sanford is an EEO/AA Employer M/F/Disability/Vet.
