We are seeking a compassionate caring and dedicatedBehavioral Health Care Managerto join our collaborative multidisciplinary Tailored Care Management (TCM) team in Lenoir and Duplin counties. designed to provide whole-person integrated health care.
What Youll Do
PORT Healths Tailored Care Management (TCM) team provides whole-person integrated health care through evaluation assessment care planning coordinating tracking and monitoring outcomes of individuals served. The Care Manager focuses on closing gaps for physical and mental health social determinants of health and other non-health related needs.This position travels within Lenoir and Duplin counties.
How Youll Benefit
Youll earn acompetitive salary between $51000 and $53000 depending on skills and experience.This full-time position offers aflexibleschedule Monday-Friday from 8 am-5 pm. We also offer competitive benefits for benefits eligible positions.
