Job Description
PORT Healths mission is to improve the lives health and well-being of individuals and families dealing with substance use and mental health disorders. We are seeking a compassionate caring and dedicatedBehavioral Health Care Managerto join our collaborative multidisciplinary Tailored Care Management (TCM) team in Lenoir and Duplin counties. designed to provide whole-person integrated health care.
What Youll Do
PORT Healths Tailored Care Management (TCM) team provides whole-person integrated health care through evaluation assessment care planning coordinating tracking and monitoring outcomes of individuals served. The Care Manager focuses on closing gaps for physical and mental health social determinants of health and other non-health related needs.This position travels within Lenoir and Duplin counties.
How Youll Benefit
Youll earn acompetitive salary between $51000 and $53000 depending on skills and experience.This full-time position offers aflexibleschedule Monday-Friday from 8 am-5 pm. We also offer competitive benefits for benefits eligible positions.

What Were Looking For
To join our team you must be willing to provide services in various community locations as needed rotate after hours support coverage knowledgeable in Microsoft Office Suite experience with Electronic Health Records software have a passion for helping others and able to link clients to community resources. We also require the following:
- A Bachelors degree in health psychology sociology social work nursing or another relevant human services area; Masters preferred
- Able to be credentialed as a Mental Health Qualified Professional
- Minimum of 2 years of experience with the mental health and/or substance use disorder population
- A valid drivers license current auto insurance and a good driving record
- Must be able to work independently have strong verbal and written communication skills
