You will be updated with latest job alerts via emailTuesday Health is seeking a compassionate skilled and experiencedLicensed Clinical Social Workerwith experience in Palliative Care to join our team. You will work alongside other members of our clinical team providing a multi-disciplinary approach to care for our members with serious illness and ensuring the delivery of high-quality palliative care services to our patients and their families.
As a Licensed Clinical Social Worker you will:
Conduct comprehensive psychosocial assessments to identify the needs concerns and preferences of patients and their families
Assist families in understanding complex care options including advanced care planning treatment goals and end-of-life decisions
Provide counseling and emotional support to patients and families dealing with serious illness addressing issues such as grief anxiety depression and coping with illness-related challenges
Collaborate with interdisciplinary team members to develop and implement individualized care plans for patients in the community setting.
Coordinate referrals to community-based services (e.g. home health hospice financial assistance programs) to meet patient and family needs
Support families in understanding and navigating decision-making processes regarding treatment options and goals of care
Provide patients and families with information and referrals to community resources financial assistance programs and support groups
Maintain accurate and up-to-date documentation of all social work interventions in the electronic medical record (EMR)
Adapt interventions to meet the diverse needs of patients from different cultural and socioeconomic backgrounds
Adhere to the organizations value-based care goals including improving patient outcomes reducing unnecessary hospitalizations and enhancing patient satisfaction
Escalate appropriate responses results findings to appropriate clinicians following the escalation procedure
Support internal and joint rounds to ensure the member experience is optimal and coordinated
Build a strong trusting relationship with every member where listening and empathy are the foundation of every interaction
Contribute to the multidisciplinary team transforming care for members with serious illness
To be successful in this role you will need:
Masters degree in Social Work (MSW) from an accredited program
Current unrestricted state licensure as a Social Worker (LCSW preferred).
Minimum of 2 years of social work experience in palliative care hospice or a healthcare setting with knowledge of value-based care models.
Experience in clinical/medical setting preferred
Experience in a multi- disciplinary care model working across network providers health plan care management support and employed clinicians
Knowledge of social determinants of health and strategies to address health disparities in underserved communities
Ability to travel to patient homes and community settings within the designated service area
Excellent communication problem-solving and interpersonal skills
Ability to multitask and work under pressure without sacrificing a positive member experience
Ability to know when to escalate and potentially act professionally in an emergent situation
Strong technical skills to navigate different documentation systems and tools; as well as ability to learn new systems as the company grows and evolves
Strong documentation skills with experience using electronic health records (EHR)
Comfort with change and ambiguity; you understand that rapid changes to the business strategy organization and priorities will come with rapid evolution of our business
