You will be updated with latest job alerts via emailKids Plus is a multidisciplinary team of nurses and social workers partnering with families and community providers to address the health and social needs of children and families experiencing homelessness. Our team specializes in medical case management while working to address the many complex needs and barriers that impact the health and well-being of children. This Public Health Nurse (PHN) is client-centered and meets families where they are to support each familys goals for well-being and stability. This PHN coordinates care offers health education and screening connects families to community resources and offers support to decrease the stress of living homeless for the entire family. This PHN also participates in community groups coalitions and advisory committees to strengthen the network of services available to familys experiencing homelessness.

Requirements:
Work Schedule:This position has a regular schedule of Monday through Friday generally 8 AM to 5 PM is a field-based nursing role and telework is not possible. This position is subject to the provisions of the Fair Labor Standards Act (FLSA) and is eligible for overtime pay. May work outside of the regular schedule to meet operational needs.
Work Location:2124 4th Avenue Seattle WA 98121.As needed the position may be deployed to other Public Health Centers and sites throughout King County.
