The Support Manager will facilitate Community Supports for people with disabilities to live within their community focusing on choice and self-determination. The Support Manager will coordinate and encourage communication between a wide variety of internal and external stakeholders ensuring a truly interdisciplinary approach to services. The Support Manager will be a liaison between all stakeholders and Fello regarding services and compliance with the person-centered plan. The Support Manager is a member of the Leadership Council and must meet the qualifications of a Direct Support Professional and complete all required trainings including Certified Medication Technician (CMT) certification.
Qualifications
The ideal candidate will have demonstrated experience supporting people with disabilities in a way that promotes independence self-determination and community engagement. They will have strong case management planning and coordination skills with the ability to build effective partnerships with stakeholders and lead interdisciplinary teams. The candidate must be a strong communicator with excellent interpersonal skills capable of coaching direct support staff and facilitating complex conversations with families providers and community partners. Success in this role also requires sound judgment adaptability in a fast-paced environment and proficiency with Microsoft Office and other technology tools.
This position requires a high school diploma or equivalent with at least five years of experience working with people with disabilities and at least two years of experience providing case management and/or planning facilitation. A bachelors degree and experience in person-centered thinking are a plus. Must have a valid Maryland Drivers License with no egregious violations.
