Community Health Worker - CHARMED Program (Bilingual Required)

Community Health Worker is represented by the Service Employees International Union (SEIU) and is eligible for overtime compensation.

The eligible list established from this recruitment may be used to fill current and future Regular (including Temporary and Fixed-term) Intermittent and Extra Help vacancies for these positions only. There are currently two (2) intermittent vacancieswithin Health Care Agency - Ambulatory Care.

An Intermittent Employee is a person employed for intermittent/temporary work on a day-to-day basis.

TENTATIVE SCHEDULE
OPENING DATE:June 13 2025
CLOSING DATE: June 27 2025 at 5:00 p.m.

Duties may include but are not limited to the following:

• Travels to meet and engage with patients in clinic settings;

• Assists with the recruitment materials and recruiting research participants;
• Collects data via surveys focus groups and interviews;
• Coordinates research operations like patient outreach and data collection;
• Coordinates research team meetings taking notes managing project timelines and ensuring follow-up of deliverables;
• Under direct supervision performs basic statistical and data analysis;
• Assists with report preparation and / or analysis for internal constituents;
• Reports on study progress to members of the study team and division;
• Presents results to members of the study team and division;
• Coordinates research study activities;
• Coordinates meetings to discuss study progress;
• Manages follow-up tasks between study meetings to ensure adherence to the study timeline;
• Communicates barriers to task completion and identifies alternative approaches;
• Collects primary research data and prepares it for analysis;
• Recruits patient participants for the research study;
• Conducts surveys and interviews with participants;

• Conducts follow-up visits or phone calls to check on patients progress remind them of appointments and reinforce care plan goals;
• Accurately documents patient interactions and progress in alignment with organizational protocols and requirements;
• Meets productivity and quality goals set by agency;
• Participates in care team huddles meetings and required training; and
• Performs related duties as required.

EDUCATION TRAINING and EXPERIENCE

High school diploma GED or equivalent PLUS any combination of education and experience which has led to the acquisition of the required knowledge and abilities.

NECESSARY SPECIAL REQUIREMENTS:

• Willing to work out in the field with at risk populations which may include adolescents people with disabilities homeless populations individuals with chronic illnesses low-income individuals and families etc.
• Must possess or obtain a valid California driver license by date of hire.
• Experience and ability to work in a Windows environment and using Microsoft Word required.
• Bilingual English/Spanish
• Strong verbal written and visual communication skills
• Attention-to-detail on the application must be demonstrated

• Experience working with diverse populations or in a similar role.
• Knowledge of public health and/or social science topics
• Experience conducting patient-facing research in a community or safety-net setting
• Acquiring knowledge of research functions
• Acquiring ability to perform research analysis duties

SUPPLEMENTAL QUESTIONNAIRE qualifying:All applicants are required to complete and submit the questionnaire for this examination at the time of filing. The supplemental questionnaire may be used throughout the examination process to assist in determining each applicants qualifications and acceptability for the position. Failure to complete and submit the questionnaire may result in the application being removed from consideration.

APPLICATION EVALUATION qualifying:All applications will be reviewed to determine whether or not the stated requirements are met. Those individuals meeting the stated requirements will be invited to continue to the next step in the screening and selection process.

WRITTEN EXAMINATION - 100%:An online un-proctored job-related written examination will be administered to test candidates knowledge of language usage interviews/confidentiality medical knowledge and terminology interpersonal relations and filing. Candidates must earn a score of seventy percent (70%) or higher to qualify for placement on the eligible list. Applicants successfully completing the examination process may be placed on an eligible list for a period of one (1) year.
