Housing Case Manager Position Summary
The Case Manager performs case management-related functions to coordinate facilitate and maintain care for the residents of Venture House and the Apartment Treatment program in Madison County.
Housing Case Manager Job Responsibilities:
- Completes assessment of housing and housing related needs.
- Works with Venture House and Apartment Treatment staff and multi-disciplinary treatment team to establish individualized service plans which meet the clients goals.
- Establishes and maintains effective relationships with other service providers: e.g. Madison County DSS SPOA Coordinator PROS Care Coordination etc.
- Acts as program liaison and primary contact for other service providers as it relates to clients coordination of services.
- Demonstrates current understanding of the Office of Mental Health regulations mental health disorders and its treatments. Understands reciprocal relationships between substance abuse and mental health.
- Assists individuals with linkage and referral to substance use disorder and mental health services where necessary.
- Responds to and effectively handles emergencies which may require flexibility of time and area of assignment.
- Completes all required documentation in timelines in accordance with program standards.
Housing Case Manager Qualifications:
Bachelors/Associates and one (1) year experience or HS Diploma and three (3) years related experience. Must have a valid drivers license and access to reliable transportation.
Pay:$21.51 per hour
