What you bring to SBHG:
- Associate degree in a social service field plus two (2) years of experience working in a mental/behavioral health field required (Internship may be substituted for a portion of the work experience) OR
- Bachelors degree in a social science field plus six (6) months of experience preferred. (Internship may be substituted for a portion of the work experience)
- Valid California Drivers License required
How you will make a difference:
- Lead and oversee the coordination of intensive case management services for BHC participants ensure timely assessments care planning and service delivery.
- Serve as a primary point of contact with court personnel including judges attorneys and probation officers and represent the program in court proceedings.
- Provide clinical and administrative guidance to case managers and support staff assigned to BHC cases.
- Facilitate and participate in multidisciplinary team meetings case conferences and court staffing to monitor progress and address barriers.
- Collaborate with program leadership on program development fidelity monitoring and quality improvement initiatives related to BHC services.
- Provides services at the TAY Center in the community and in TAYs homes. Uses personal vehicle to drive within the community to provide services and to transport TAY and/or TAYs family members in personal vehicle.
- Provides case management services to clients and their families as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team.
- Participates in meetings such as Treatment Team Meetings to ensure clients needs are met and recommendations are followed through.
- Documents all services provided clearly and concisely within the Electronic Medical Record within the timelines and framework established by Stars.
