Home Health Licensed Practical Nurse (LPN)

Pay:$32.51- $38.88 per hour plus mileage reimbursement of 70 cents per mile
Schedule: Monday -Friday 8a-5p
Territory: Northwest Metro area (as far as Delano or Big Lake)
As a Home Health Licensed Practical Nurse heres a big-picture view of what youll do:
- Provide home-based care to patients with illnesses injuries and chronic diseasesoften after hospital dischargeunder the supervision of an RN
- Work with a team of physicians RNs home health aidesand therapists to deliver quality patient care
- Take patient vitals do assessments document progress and report changes to RN supervisor
- Assist RN in carrying out patients plan of care and teaching patient self-care techniques
- Educate family on the patients plan of care medication and home safety
- Collaborate with interdisciplinary team to coordinate patient care and plan for discharge
- Communicates with patients regarding scheduling visit times
- Typically see about 6 patients/ day
Requirements:
- Current LPN license in the state of Minnesota
- CPR certification
- One year of home care experience is preferred
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing strong communication skills and compassion
- Clean driving record
- Ability to pass a criminal background check
- Experience with both wound care and catheter care
