Job Overview:
The Part-Time Rapid Rehousing Case Manager is a key member of the HELP Program team supporting young adults ages 1724 in achieving housing stability and workforce readiness. This role combines direct case management workshop facilitation and administrative support. The Case Manager will carry a small caseload assist in the coordination and delivery of life skills and employment readiness workshops and ensure accurate documentation and compliance with program requirements. Key Responsibilities:
Case Management & Participant Support:
- Carry a small caseload of participants providing individualized support around housing and employment goals.
- Assist participants in applying for housing and employment opportunities.
- Monitor participant progress and provide follow-up services to promote stability.
- Support participants in building financial literacy and independent living skills.
Workshop Facilitation:
- Assist in planning and facilitating workshops related to employment readiness life skills and housing education.
- Encourage participant engagement and track attendance and outcomes from group sessions.
- Collaborate with HELP team members and external partners to deliver high-quality content.
Administrative & Program Support:
- Maintain accurate and up-to-date participant files in accordance with program and funder requirements.
- Complete case notes progress updates referral documentation and check requests in a timely manner.
- Assist with enrollment eligibility screening and intake documentation for new participants.
- Ensure confidentiality and proper handling of all participant information.
- Track expenses and support compliance with program guidelines including grant-specific requirements.
Collaboration & Communication:
- Communicate regularly with the HELP Program Manager financial consultant and support staff to coordinate services.
- Attend staff meetings supervision sessions and trainings as required.
- Stay informed about city and state housing laws local resources and workforce trends.
Skills & Qualifications:
Required:
- Associate degree in social work Human Services Psychology or a related field.
- At least one year of experience in case management youth services workforce development or housing support.
- Strong documentation and organizational skills.
- Comfort facilitating group workshops and engaging young adults.
- Excellent interpersonal and communication skills.
- Ability to work flexible hours including some evenings and weekends.
- Ability to travel within Philadelphia and nearby communities.
- Culturally responsive and able to work with diverse populations.
Preferred:
- Familiarity with Philadelphias housing system and workforce development resources.
- Experience working with grant-funded programs and adhering to compliance requirements.
- Knowledge of financial literacy or independent living skills training.
