RN Case Manager - Full-Time

Schedule: Full-Time M-F 8am - 5pm (can be flexible).
Location: Northwestern Suburbs of Minneapolis MN. Relatively large service area including NW Metro suburbs such as Fridley Brooklyn Park Maple Grove Medina Delano St. Michael/Albertville Big Lake Elk River Oak Grove Anoka Dayton MN. (Mileage reimbursement will be provided at $0.70/mile!)
Pay rate:Up to $51.67 depending on experience!
*This is a temporary assignment that is expected to be temp-to-hire!
Position ResponsibilitiesInclude:
- Perform assessments/follow up assessments and procedures match employee skills with the clients needs and develop working relationships with discharge planners and social service workers.
- Obtain physicians plans of treatment and initiate execution and provide services ordered by the physician as indicated in the care plan.
- Counsel and educate the patient/family in meeting healthcare related needs and effectively communicate with members of the interdisciplinary healthcare team.
- Complete timely and accurate documentation/clinical notes including addressing the patients progress.
- Supervise paraprofessional and licensed professional team members (LPNs).Secure equipment and supplies and coordinate placement for use in patients homes and refer patients to other services as needed.
- Participate in Quality and Performance Improvement (QAPI) program along with the Offices Emergency Preparedness program as requested.
- Follow accepted standards of nursing practice and use clinical decision making to efficiently and effectively manage an assigned patient population.
- Access personal health information (PHI) necessary to fulfill the above duties and responsibilities.
- All other duties as assigned.
Required Skills/Qualifications:
- An active RN license in the states of MN.
- Two years of current RN experience is preferred.
- One year of RN Case Management experience is preferred.
- Must have reliable transportation for patient visits. (Opportunity for company vehicle available!)
