Job Description
Schedule: Full-Time Monday Friday 8 AM 5 PM
Setting: Community-based care outpatient-focused Model: PACE (Program of All-Inclusive Care for the Elderly)
At WelbeHealth were redefining how healthcare is delivered to our most vulnerable seniors. As a Primary Care Physician (PCP) Pod Leader in our value-based PACE program youll deliver longitudinal team-based care to a small panel of medically complex seniors-focusing on quality of life not quotas.
Unlike traditional healthcare models we empower you to spend more time with each participant develop deep patient relationships and work collaboratively within an interdisciplinary care team.
What Youll Do-
Serve as the clinical leader for a pod of patients and interdisciplinary team members

Deliver comprehensive person-centered care to frail seniors including home visits and clinic visits

Collaborate with nurses social workers therapists and other specialists to develop and implement care plans

Participate in a rotating call schedule (phone-based only a few evenings/weekends per month)

Engage in leadership development with potential to advance to Medical Director roles
