Job Description

Working title: Behavioral Health Systems Specialist

Classification title: Prevention Health Specialist

Division: Public Health Services

Program: Community Health Improvement (Healthy By Design)

Reports to: Healthy By Design Coordinator (Program Coordinator 2)

FLSA status: Non-Exempt: Full-time

Schedule: Monday-Friday; 8am-5pm

Wage Range: $20.48 to $26.06 hourly; depending on number of years of transferrable experience and internal equity

Division and Program Overview:

Health Promotion Services of RiverStone Health exists to improve the life health and safety of Yellowstone County residents at the population level. Services include the prevention of poor health outcomes and health behaviors through assessment health education community partnerships policy systems and environmental strategies.

TheBehavioral Health Systems Specialist is responsible for developing and implementing tools and systems that support coordination of services and referrals among area agencies and care coordinators such as referral pathways reimbursement models and more. This position is responsible for the care coordination innovation component of the crisis diversion grant for Yellowstone County. Thispositionwill work with community partners including area community health workers navigators care managers and like professions to improve behavioral and other health outcomes for residents of Yellowstone County. This project will be implemented in partnership with the Healthy By Design Coalition of Yellowstone County and the to be named Behavioral Health Systems Improvement coalition formally known as Substance Abuse Connect.
