The University of Miami Health System UHealth Department of Pediatrics- Infectious Disease has an exciting opportunity for a Full-Time Case Worker 2 to work in Miami.
The Case Worker 2 monitors outcomes data collection and record keeping and coordinates services for the respective population including providing information about professional development opportunities quality improvement strategies and counseling to improve the clients social and psychological conditions. The Case Worker 2 assists those experiencing difficult situations by helping them find the community resources/services that they need contributes to the development of family support plans and monitors its implementation and modification.
Core Responsibilities:
Conducts situational analysis which may be at home and/or office visits mentors and provides technical assistance as needed.
Collaborates with the service agencies and program leadership to provide and administer services appropriately.
Develops and provides training to family support networks and/or care providers on various topics related to quality early care and education.
Maintains and reviews records and files to ensure compliance with project guidelines.
Collaborates with all funding sources to ensure the availability of adequate resources.
Maintains current knowledge of services available and changes applicable rules and regulations.
Attends meetings and training which may include the Early Learning Coalition United Way and the Quality Counts Career Center to enhance developmental potential.
Adheres to University and unit-level policies and procedures and safeguards University assets.
Department Specific Functions:
Ensure patients are enrolled in insurance and free or low-cost social service programs for which they are eligible (e.g. food stamps WIC housing SSI Medicaid/Medically Needy Program Ryan White and ADAP etc.).
Learns the requirements of the new Affordable Care Act and the enrollment process to ensure patients know how to appropriately enroll or transition their care from Ryan White to a health care exchange or other insurance program that will allow them to maintain their care with our program. Identifies and resolve real or perceived barriers to care (e.g. disclosure cultural beliefs denial transportation childcare support systems functionality insurance etc.) to enable patients to adhere to care recommendations and improve their health outcomes.
Assists with patient-provider communication. Provides clients with information on available clinical trial opportunities and facilitates linkages to research staff.
Coordinate the transition of youths from pediatrics to adult care providers using agency approved protocols.
Assist with Coordination of Screening Clinic appointments for newborns exposed to HIV including tracking births and completion of screening protocols.
Educates and counsels patients around issues related to their illness and/or life situation and assists in finding additional information from websites and agencies.
Maintains knowledge of community health and welfare resources and the variety of programs offered.
Documents filing and actions taken according to departmental guidelines and standards.
Assists with record audits for documentation and record compliance.
Assist with data entry in Careware System.
Disseminate and track various surveys required by various funding sources.
Maintain a log of patient resources dispersed from various grant sources.

CORE QUALIFICATIONS:
Bachelors in social work or related field.
Minimum 3 year of relevant experience
Haitian Creole speaking preferred
Skill in completing assignments accurately and with attention to detail.
Ability to analyze organize and prioritize work under pressure while meeting deadlines.
Ability to process and handle confidential information with discretion.
Ability to work evenings nights and weekends as necessary.
Commitment to the Universitys core values.
Ability to work independently and/or in a collaborative environment.
Any appropriate combination of relevant education experience and/or certifications may be considered.
