Job Description

Summary:

Supervises the daily staff activities of Social Worker staff in addition to their normal social worker responsibilities. The Social Worker Supervisor provides patient education and emotional support is responsible for referrals to community agencies coordinates patient care with other team members and assists with concrete services.

Responsibilities:

Responsible for programs patients social needs including discharge planning for inpatient and outpatient. Responsible for evaluations to include psychosocial assessments in order to determine appropriate candidacy and continues to work with patient and family. Provides ongoing assistance with social needs to all patients. Responsible for the presentation of patients at selection committee as required by CMS where staff responsibilities to fulfill required service levels. Leads the handling and resolution of complex issues and quality of services delivered and facilitates staff development programs. Ensures staff compliance with departmental and organizational policies procedures and direction orientation training coaching and mentoring to staff. Performs or assists with performance evaluations and disciplinary and is a resource to specialty social workers.

Masters: Social Work (Required)Fingerprint Screen (FINGERPRNT) - Florida Department of Childrens & Family (DCF)

Complexity of Work: Requires critical thinking skills decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate Work Experience: Five (5) years Information: Additional Information: Employment in this position is contingent upon successful completion of a Level II Criminal History record Certification Info: Certified Clinical Transplant Social Worker (CCTSW) credential preferred where applicable. LCSW participation in social work professional organizations preferred.
