Short Summary: Under the direction of the Behavioral Health Unit Director and the Clinical Nurse the social worker provides psychosocial services to patients and families of all ages with particular emphasis on the emotional and social issues resulting from illness disability and/or death. These duties are to be performed in a highly confidential manner following the mission values and behaviors of Insight Hospital and Medical Center and the attributes of Careful Nursing. Employees are further expected to provide a high quality of care service and kindness toward all patients staff physicians volunteers and guests. Job Responsibilities: - Under the direction of the UR/CM/CDI Director the Behavioral Health Case Manager has the functional responsibility for ensuring that a comprehensive review of patients needs are performed and that the Case Manager creates a discharge plan in conjunction with the Licensed Social Worker assigned to the patient.
- He/she is responsible for team and individual development utilization of human technical and material resources and quality improvement activities at the unit and organization level.
- Communicate daily with admissions personnel regarding admissions and discharges to Behavioral Health Unit
- Addresses referrals from providers including referrals via warm hand-offs.
- Initiate ongoing communication with the resident and residents family to assess discharge needs.
- Serving as a liaison between the client and social financial health and legal services
- Communicate with physicians to ascertain their plans for a timely discharge.
- Document discharge planning as an ongoing review daily
- Makes notes within the EMR (Cerner) in regards to care coordination needs of the patient.
- Knowledgeable of patients financial status diagnosis and discharge needs.
- Responsible for home care needs being met by the time of discharge with a goal of arrangements completed 24 hours prior to discharge when date of discharge is known.
- Cooperate with insurance companies based on information received.
- Participates in IDT rounds and daily huddles to assess needs and coordinate services.
- Developing a plan of action to improve the clients well-being and continuity of care post-discharge and documents as such.
- Assist as needed with obtaining referrals prior authorization for Home Health Care and DME for home use by the patient.
- Communicates and provides information to SNFs Acute Rehabs SARs and LTAC/LTACHs to ensure that patient obtains placement if needed before discharge.
- Demonstrates cultural competency and commitment to patients.
- Demonstrates commitment to the philosophy of the organization.
Requirements: - A Masters Degree in Social Work from an accredited school is required.
- Licensed as a Social Worker in the State of Illinois required.
- Knowledge of patients growth and development needs.
- Ability to maintain a high degree of accuracy in carrying out work duties.
- Ability to cope with high stress situations.
