Job Description
The WV Intensive Clinical Care Coordination Team ( WVICCC ) will focus on youth who are out-of-state in group residential facilities and psychiatric residential treatment facilities. The goal of the team is to bring youth back to WV from out-of-state into community level services that are effective in keeping the youth at home and not returning to a higher level of care. The team will make recommendations assist with the development of the discharge plan and implementation of that plan. This team will work with the out-of-state providers DHHR case workers and community partners to accomplish this. The Clinical Specialist will work with a team to identify all needs of the child and develop and implement discharge plans for the children described above. The Clinical Specialist will be responsible for completing the Child and Adolescent Needs Assessment ( CANS ) and the Child and Adolescent Functioning Assessment ( CAFAS ). Applicants can be located anywhere in WV or bordering states but must have worked with WV youth. The position is remote. Duties and responsibilities of the position: Develop and implement the discharge plan for each child. Complete the Child and Adolescent Needs Assessment ( CANS ) and the Child and Adolescent Functioning Assessment ( CAFAS ) Work with Out-of-State Providers DoHS Aetna to secure services for children by discharge Follow-up with families after discharge to reduce recidivism Participate in OOS treatment teams Collect required data and reports Other duties as assigned
