You will be updated with latest job alerts via emailWe are seeking a compassionatePalliative Care Case Coordinatorto join our team in our WA Head Office in West Perth on an 18-month Full-time fixed term contract.
About the Role
Reporting to the Chief Operating Officer you will be responsible for implementing the Palliative Aged Care Outcomes Program (PACOP) delivering and coordinating in house education sessions within our Residential Aged Care Homes (RACH) for all clinical and non-clinical staff. This role will require you to support the primary care coordination between general practices and RACHs to improve our residents experience. You will also be responsible for undertaking quality improvement activities aligned with the outcomes of this project and reflective of the Aged Care Quality Standards and we will require you to undertake collaborative sessions with General Practitioners (GPs) local palliative care services and external agencies.
Your well-honed communication assessment and problem-solving skills are easily demonstrated when leading inter-disciplinary teams as well as providing relationship-centred care that ensures the dignity and respect of all residents.
