Job Description
Join our team! Sonora is the leading Behavioral Health hospital in Tucson providing services to adults and children. The Discharge Planner works closely with the care treatment team of psychiatrists nurses social workers and techs. Provides assistance to patients and their families by developing a documented continuing care plan to ensure ongoing support in recovery and or other behavioral mental or health care follow up after discharge from the hospital.
- Based on patients needs develop a continuing plan that promotes their long term health and relapse prevention.
- Responsible for setting discharge appointments including group or individual therapy psychologist or psychiatrist.
- Make arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care.
- Ensure all documentation regarding patient discharge planning is clear concise and timely.
- Act as a liaison between referral sources patients and counselors.
