Job Description:
The Housing Resource Navigator is a highly responsible professional position providing outreach assessment care management advocacy and referrals/linkages to needed services and resources at the Our Promise Drop-in Center on the Navigating Hope Mobile Outreach Van and within the community.
The position will provide care management across the Navigating Hope and Coordinated Entry Programs.
- Navigating Hope is a partnership between Family Promise and Morris County Human Services that utilizes an outreach van to provide care management and a variety of services including Single Stop screenings for low-income and homeless individuals in the van. The van targets hard-to-reach communities and populations and is intended to fill traditional gaps in community service provision. Navigating Hope outreach is also a tremendous asset during Code Blue emergencies (extreme weather events and extreme cold conditions) focuses on prevention and diversion from a homelessness crisis and accesses some of Morris Countys most vulnerable community members with direct support.
- Coordinated entry is the process that helps ensure that all people experiencing a housing crisis in Morris County have fair and equal access and are quickly identified assessed for referred and connected to housing and homeless assistance based on their needs and strengths no matter where or when they present for services. It uses standardized tools and practices incorporates a system-wide Housing First approach participant choice and coordinates housing and homeless assistance such that housing and homeless assistance is prioritized for those with the most severe service needs.
The position is full time and will report to the Director of Outreach & Crisis Intervention. The position will work as part of a dynamic innovative cross systems team that embraces a creative environment.
Duties (Navigating Hope Coordinated Entry and Code Blue):
- Staff the Navigating Hope Mobile Outreach Van along with county Human Services staff
- Crisis intervention
- Ensure continued stock of van necessities
- Connection to resources and services for those using warming centers during Code Blue season
- Perform clear and concise intakes assist in client goal setting coordinate appropriate services and ensure access to needed resources including shelter and transportation provide advocacy
- Maintain confidential client records review case notes follow up with/track client progress discharge/terminate clients appropriately
- Be a positive role model offering support and guidance in life skill development
- Maintain professionalism
- Provide resource navigation through county services and systems
- Use Single Stop technology to provide benefits eligibility screenings application assistance and report on the programs benefit outcomes
- Liaison with county service providers
- Work collaboratively with the other program workers to ensure positive client outcomes
- Report data collection responsibilities include:
- Trip/Event Dashboard Data
- HMIS Reporting
- Satisfaction Survey implementation and reporting
- Maintain overall confidentiality standards

Skills/Qualifications/Requirements:
- BSW or MSW LAC LPC or Related
- Bilingual a Plus
- Experience Working Directly with Low Income or Homeless Families Preferred
