Your Specific Responsibilities:
Responsible for the coordination of the Family and Personal Health Teams which includes Heathy Start Healthy Babies and the Maternal Child Block grant. This position spends the majority of the time engaged in supervisory responsibilities. This includes communicating with motivating training and evaluating employees in addition to planning and directing employees work. The incumbent has the authority to hire transfer suspend lay off recall promote discharge assign reward or discipline subordinate employees or effectively recommend such action. The incumbent will also be required to develop performance expectations meet with staff regularly to discuss performance complete performance reviews maintain vacation and work schedules ensuring proper coverage and complete timesheets. Contributes to the organizations culture of performance excellence by actively seeking to improve personal and organizational performance through measurement analysis knowledge management and processes improvements and participation in performance management committees councils and/or workgroups. Continuously demonstrates the Departments ICARE values. This position reports to the Assistant Community Health Nursing Director.
Family and Personal Health Team Supervision and Tasks
- Establish and oversee administrative procedures to meet objectives set by Healthy Start Coalition or senior management.
- Direct activities of professional and paraprofessional staff members.
- Evaluate the work of staff to ensure that programs are of appropriate quality and that resources are used effectively.
- Participate in the determination of organizational policies regarding such issues as participant eligibility program requirements and program benefits.
- Prepare and maintain records and reports such as programmatic reports budgets personnel records or training manuals.
- Provide direct service and support to individuals or clients such as handling a referral for WIC child advocacy issues conducting a needs evaluation or resolving complaints.
- Establish and maintain relationships with other agencies and organizations in community to meet community needs and to ensure that services are not duplicated.
- Recruit interview and hire or sign-up volunteers and staff.
- Research and analyze member or community needs to determine program directions and goals.
- Implement and evaluate staff volunteer or community training programs.
- Speak to community groups to explain and interpret agency purposes programs and policies.
- Plan and administer budgets for programs equipment and support services.
- Coordinates the activities of the Family and Personal Health Teams which include Healthy Start Healthy Babies Nurse Family Partnership and the Maternal Child Block grant.
- Works collaboratively with Epidemiology; HIV; Hepatitis; and Women Infants and Children Departments for support of pregnant women and infants.
- Attends required meetings and maintains an active presence in community groups working to reduce the incidence of infant mortality low birth weight and increase developmental outcomes in infants.
Attends Healthy Start Care Coordination Manager Meetings
Attends Healthy Start Board Meetings
Attends other community meetings as approved by the supervisor.
- Coordinates team activities by assigning client caseloads holding monthly team meetings and providing one on one assistance to team members as needed.
- Submits monthly and quarterly reports for the Healthy Start contract and Maternal Child grants as required.
- Works with the Assistant Director of Nursing to manage the Healthy Start budget.
Healthy Start Case Management
- Provides care coordination services to eligible Healthy Start families according to the Healthy Start Guideline.
- Promotes basic health principles and preventative care related to disease control and maternal/child health.
- Provides appropriate referrals to client for community resources.
At Risk Population
- Collaborates with the Department of Children and Families and other community partners in the follow-up of referred at risk families.
- Oversees classes in area high schools to the pregnant and teen mother populations.
- Oversees Parenting classes.
School Health Support
- Provides School Health support services as requested if staff is available and will not affect Healthy Start services.
Quality Improvement
- Attends Quality Improvement meetings.
- Works on Community quality improvement initiatives as assigned.
Other
- Enter Employee Action Report System into Health Management System according to agency protocols and timeframes.
- Attend and complete required in-services and trainings.
- Other duties as assigned.
Required Knowledge Skills and Abilities:
- Ability to supervise staff
- Proficiency in use of computers especially Microsoft Office applications and working with data management systems
- Skilled in-Home visiting experience
- Ability to deliver health education services
- Skilled in organizing Planning and Prioritizing Work Developing specific goals and plans to prioritize organize and accomplish your work.
- Ability to Communicate with Supervisors Peers or Subordinates Providing information to supervisors co-workers and subordinates by telephone in written form e-mail or in person.
- Skilled in getting Information Observing receiving and otherwise obtaining information from all relevant sources.
- Ability to plan organize and coordinate work assignments independently
- Ability to communicate effectively clearly and concisely through oral and written means
- Ability to establish and maintain effective working relationships with others

Qualifications:
REQUIRED:
- Incumbent may be required to work before during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters or performing other emergency duties including but not limited to responses to or threats involving any disaster or threat of disaster man-made or natural.
- Greater than 3 years of Maternal/Child Health experience
- Valid Drivers License and reliable transportation.
