General Description
Meets Agency Mission by providing administrative support in facilitating nutrition services for consumers in the Title XIX Program.
Essential Functions
- Facilitate program services delivery for consumers in the Title XIX Program
- Provide administrative support for case managers as it relates to the Home Delivered Meals Program
- Facilitate confidence in Administrative Assistant 1 and therefore the Agency and programs
- Protect consumers and reduce Agency risk
- Provide excellent customer service in a professional manner
1. Facilitate program services delivery for consumers in the Title XIX Program
- Provide database support by verifying entering and updating data related to Home Delivered Meals Program participation and run reports
- Verify accuracy of Home Delivered Meals count in agency databases to ensure accurate billing
- Enter retrieve and track program statistics and other data for Title XIX consumers
- Coordinate and expedite Home Delivered Meals Program paperwork flow through the Unit
- Compose documents letters reports etc. and complete general office work such as copying filing file preparation etc.
2. Provide administrative support to case managers as it relates to the Home Delivered Meals Program
- Monitor and process program referrals and forms to initiate change suspend and terminate Home Delivered Meals service
- Review and verify accuracy and completeness of participant information and support documentation research multiple agency databases to obtain and enter missing data
- Communicate with Title XIX Program staff to verify consumer eligibility for services; communicate and resolve system discrepancies; complete forms to initiate change suspend and terminate services
- Monitor and track case actions related to home delivered meals services follow up and escalate as needed.
3. Facilitate confidence in Administrative Assistant 1 and therefore the Agency and programs

4. Protect consumers and reduce Agency risk
- Follow policies regulations and requirements of program and Agency
- Provide documentation as set forth by Federal State funding regulations and Agency policy
- Serve as a Mandatory Reporter of suspected abuse of vulnerable populations as required by policy and regulation.
- Maintain and share information according to privacy policies and regulations.
