Job Summary:
We are seeking a skilled and compassionate Registered Nurse (RN) to join our team to do Oasis assessments and Visits providing direct patient care assess patient health needs and develop care plans in coordination with physicians and other healthcare professionals.
We are seeking a skilled and compassionate Registered Nurse (RN) to join our team to do Oasis assessments and Visits providing direct patient care assess patient health needs and develop care plans in coordination with physicians and other healthcare professionals.
Key Responsibilities:
- Conduct initial and follow-up patient assessments in the home setting.
- Develop implement and revise individualized care plans.
- Administer medications and treatments as prescribed.
- Educate patients and their families about their care plans medications and lifestyle adjustments.
- Monitor and document patient progress in accordance with agency policies and regulatory guidelines.
- Coordinate with physicians therapists and other healthcare professionals to ensure optimal patient care.
- Adhere to infection control and safety protocols.
- Participate in on-call rotations as required.
Qualifications:
- Active Registered Nurse (RN) license in Florida.
- Minimum of 2 years of nursing experience (home health experience preferred).
- Current CPR certification.
- Valid drivers license and reliable transportation.
- Strong organizational and communication skills.
- Ability to work independently and make sound clinical decisions.
